A 71-year-old man with osteoarthritis and diabetes mellitus type 2 saw his primary care physician because his left knee was painfully swollen and warm to the touch. He was unable to swing his leg freely. The pain came on suddenly overnight, and he could not recall any injury that could explain the pain or swelling. He did not have a fever or any other systemic symptoms.
Could it be septic arthritis? Bursitis? Calcium pyrophosphate deposition disease? Polymyalgia rheumatica?
