Fitness Buff Sidelined by Knee Pain – Let’s Diagnose

A 25-year-old man visited his doctor with a complaint of anterior right knee pain, which he had been experiencing for the past 2 months. He reported no recent falls or injuries. He started a CrossFit program 5 months ago but discontinued his participation due to anterior knee pain with deep squatting and decreased range of motion. On examination, there was pain with patellar grind and crepitus with knee range of motion. Patellar instability was not found. Plain x-rays of the knee revealed preserved joint space without any acute fractures or malalignment.

What’s the diagnosis?

  1. Patellar tendinopathy
  2. Iliotibial band syndrome
  3. Knee plica syndrome
  4. Patellofemoral pain syndrome
