Transgender Man with Fever, Truncal Eruption – Let’s Diagnose

A 17-year-old transgender man visited his doctor reporting fever, multiple joint pain, and a progressive truncal eruption that had begun a few weeks prior. On examination, there were multiple erythematous papules and nodules that primarily covered his trunk and were very painful. The patient’s mother shared that her son had been on hormonal therapy with testosterone for 6 months. She also mentioned that the patient’s father had a history of severe acne during adolescence.
